What the referral form covers


These are the referral reasons printed on the form above.

  • BPPV / Vertigo

  • Vestibular Hypofunction

  • Vestibular Migraines

  • Dizziness

  • PPPD

  • Impaired Balance

  • Meniere’s Disease

After you refer

What your patient can expect


  • You receive a report


    Your patient’s initial assessment includes a comprehensive report, which is sent to their GP and/or specialist.

  • Allow 60 minutes


    Initial appointments run approximately 60 minutes. Patients are asked to bring any referrals, test results or doctor letters relevant to their condition.

  • What the assessment involves


    A thorough history, then physical assessment that may include balance tests, eye movement tests, head movement tests and positional tests. Treatment may include a repositioning manoeuvre, balance exercises and gaze stabilisation exercises. No joint manipulations are ever performed.

  • A referral is not required


    Many patients come directly from their GP or medical specialist, but a referral is not needed to attend — patients can call reception and book an initial appointment themselves.

  • Medicare and health funds


    Treatment is generally not claimable with Medicare, and is claimable only where you have provided an Enhanced Primary Care (EPC) / Chronic Disease Management (CDM) plan. We do not bulk bill, so a fee applies in addition to the Medicare plan. Patients with physiotherapy extras cover may be able to claim — initial consultation item code 500, follow-up 505.
